DIRECT PRIMARY CARE MEMBERSHIP
MEMBERSHIP INCLUDES
- Access: Unlimited primary care visits, unlimited messaging with your care team, and same- or next-day appointments.
- Comprehensive primary care: Preventive and wellness care, acute and sick visits, chronic disease management, medication management, women’s and men’s health, mental health support, and personalized lifestyle guidance.
- More value: Select point-of-care testing, care coordination, referrals, review of outside testing and records, and access to discounted cash pricing on select services.
- Beyond membership: Laboratory testing, imaging, medications, specialist visits, and other services provided outside of Fairway Health are not included in the membership fee. Patients may use their health insurance for these services when accepted by the outside provider, or our team can help identify affordable self-pay options when available.
MEMBERSHIP DETAILS
- 12-MONTH MEMBERSHIP: DPC membership begins with a 12-month commitment. Monthly billing is available for convenience. Early cancellation may be considered for approved extenuating circumstances.
- MEMBERSHIP, NOT INSURANCE: Fairway Health membership is not health insurance. We recommend maintaining insurance for emergency care, hospitalizations, specialists, imaging, and other services outside the membership.
- SIMPLE MONTHLY BILLING: Your membership fee is automatically billed monthly and reserves your place in our limited patient panel, whether or not you need a visit that month.
- OUTSIDE SERVICES: Outside labs, imaging, prescriptions, specialist care, hospital services, and other third-party services are not included. Members may have access to discounted cash pricing or may use insurance when applicable.
- DIRECT ACCESS, NO PER-VISIT FEES: There is no additional visit charge for included DPC services. Members have unlimited primary care visits and non-emergency provider messaging, subject to appropriate scheduling and clinical availability.
- AFTER YOUR FIRST YEAR: Following the initial 12-month term, membership continues month-to-month and may be canceled with 30 days’ written notice.
GLP-1 WEIGHT MANAGEMENT PROGRAM
Starting at $250/month — medication included
HSA and FSA funds may be used for eligible expenses. Eligibility and reimbursement depend on your individual plan.
Treatment includes semaglutide or tirzepatide when clinically appropriate.
